Before Your Appointment

Before Your Appointment

Complete the requested information

Forms may ask about development, communication and social interaction, attention, activity and impulsivity, behaviour and everyday functioning, and your child’s strengths and difficulties.

School or nursery information

School/nursery information is an important part of the assessment. For ADHD assessments, this may include a teacher SNAP-IV questionnaire.

Useful existing documents

School reports

SEND support plans

EHCP, if applicable

Previous assessment reports

Speech and Language Therapy reports

Other relevant professional reports

You do not need to obtain new professional reports specifically for the assessment.

Returning your forms

Completed parent and school information should be returned securely to the clinic using the instructions provided with your assessment forms. With parental consent, school or nursery information may be requested directly from the educational setting and returned directly to the clinic. Please return all requested information before the assessment wherever possible, as this allows the clinical information to be reviewed in advance.

Preparing your child

There is no need to coach your child or practise assessment activities. A simple, age-appropriate explanation that they are meeting someone to understand how they think, communicate, learn and manage everyday situations is usually sufficient.

On the Day

Bring or use any glasses, hearing aids or communication aids your child normally needs. Your child should take their usual prescribed medication unless you have specifically been advised otherwise.

Please return the requested parent and school information before the assessment wherever possible. If there is insufficient information to reach a reliable clinical conclusion, further information or assessment may be required before the assessment can be concluded.
