Explain that school/nursery information may cover communication and social interaction, attention, activity and impulsivity, learning and organisation, behaviour/emotional regulation, friendships, strengths, support already provided and response to interventions.
Once your child’s assessment has been accepted, we will provide the appropriate parent and school forms and explain how they should be completed and returned securely.
The forms provided will depend on whether your child is having an autism assessment, ADHD assessment or combined assessment.
Please use the forms sent specifically for your child’s assessment.
Completed parent and school information should be returned securely to the clinic using the instructions provided with your assessment forms.
With parental consent, school or nursery information may be requested directly from the educational setting and returned directly to the clinic.
Please return all requested information before the assessment wherever possible, as this allows the clinical information to be reviewed in advance.
Public downloads may include only general guidance such as:
Information for Parents – Preparing for Your Child's Assessment
Information for Schools & Nurseries – Providing Information for an Assessment
Do NOT make patient-specific assessment forms, SNAP-IV forms supplied for a case, ADI-R, ADOS-2 or ACE materials publicly downloadable.
